Case Manager, Street Outreach

Classification: Contract Fulltime
Hours: 35 hrs/wk.
Range: $50,886 to $73,774
Location: 185 Carlton St. Toronto

Position Summary
The Tikinagan Street Outreach is a new program. We are looking for an individual that has experience working with those that are visibly and invisibly homeless to help build and develop this new program. The Tikinagan Street Outreach Case Manager will provide brief case management support to those that are street involved and affected
by homelessness.

Major Responsibilities
Under the direction of the Supervisor, the Tikinagan, Street Outreach Case Manager will:
• Provide pre/postnatal brief case management support
• Provide addiction support and utilize harm reduction strategies that align with Indigenous culture and practices
• Support those with concurrent disorders and provide referrals to additional supports when needed
• Provide flexible, mobile and accessible supports through street outreach initiatives.
• Provide crisis intervention
• Provide support and safety planning for those impacted by family violence
• Maintain and encourage ongoing networking with health and social community services
• Address the social determinants of health with individuals who face many complex barriers.
• Build relationships and provide brief supportive counselling.
• Work within a diverse multi-disciplinary and integrated team and support the program when required
• Ensure community members are provided with the appropriate referrals and immediate resources according to their needs.
• Maintain ongoing program documentation, submit reports, records and statistics.
• Participate and plan in the development of an effective street outreach program.
• Implement culturally aware and trauma informed best practices
• Provide support with preparing resources/survival packs and assist with the distribution
• Other Duties as assigned

What we are looking for
• Degree/ Diploma in the Human Service or Social Services field and 2-5 years of experience in a related position. Alternatively, an equivalent combination of relevant education in a related field and experience working with the Aboriginal community may be considered. Special consideration to those with experience with marginalized, homeless, or concurrent disorders populations.
• Well developed crisis intervention, prevention, and problem-solving skills.
• Excellent understanding of harm reduction, and psychosocial rehabilitation principles, with the ability to effectively utilize these principles.
• Excellent knowledge of Aboriginal trauma informed best practices
• Knowledge of Aboriginal and mainstream community resources helpful and accessible to the street population.
• Knowledge of the unique dynamics that exist within the First Nations, Inuit, and Métis cultures
• Strong time management skills, interpersonal skills and written and verbal communication skills.
• Excellent computer and technical skills.
• Solid counselling, negotiation, conflict resolution, and people management skills.
• Current vulnerable sector police record check, or ability to obtain one.
• Initiative and respect for confidentiality.
• Strong knowledge of pregnancy, birth, infant mental health, early infant care and birth year cultural teachings an asset, but not required.
• Must have a driver’s license, access to a car and hold an insurance policy that meets agency’s standards
• Comfortable driving a large van (no special license required)
• Must be comfortable with light lifting and bending
• Demonstrated understanding, and commitment to, integrating the Native Child and Family Services of Toronto Mission and values into practice, service and relationships.
• Demonstrated understanding of workplace Health and Safety practices and understanding of an employee’s responsibility under current legislation.
• Ability to work effectively with all levels of staff, to maintain effective communication and working relationships, demonstrating strong interpersonal skills, tact, sensitivity and build strong internal relationships.
Salary$50,886 - $73,774
