| Do you have experience of Primary Care Nursing and supporting clients living with chronic disease? Do you have a passion for working with vulnerable clients? If so, we would love to chat with you about our new casual Registered Nurse opportunity in the Liver Care Clinic based in downtown Kamloops. As the Primary Health Care Nurse at our clinic, you will be responsible for providing comprehensive and coordinated nursing care to clients living with chronic disease in an outpatient clinic through the assessment, planning, implementation and evaluation of care delivery. The Primary Health Care Nurse assists individuals to achieve safe, realistic and reasonable healthcare goals and to maintain optimal health through the provision of information/health promotion, disease prevention and treatment/care management services. Working within a chronic disease management model, the Primary Health Care Nurse is responsible for working in direct collaboration with an inter-professional care team and defined network of Physicians, Mental Health, Pharmacy and other specialist resources to conduct integrated care planning, offer client assessments/clinic visits, and provide ongoing monitoring and self-management support follow-up for clients. This is a casual position which means hours may vary and are not guaranteed. Shift times are Monday to Friday 08:18 – 16:30. Apply today and join our team! | |
| Credentials | Current practising registration with the College of Registered Nurses of BC (CRNBC). Current valid BC Drivers License and use of a personal vehicle for occasional business use. |
| Education Requirements | Baccalaureate degree in Nursing Post graduate courses in a chronic disease specialty or advanced skills training in Hepatology, such as the BCIT Viral Hepatitis course. |
| Experience | Three years recent related clinical nursing experience in chronic disease management of hepatitis and/or other blood borne diseases; or an equivalent combination of education, training and experience. |
