Position Overview:
SHN is committed to improving the health and access to culturally safe healthcare of Indigenous people in our community, and we recognize that we all have a contribution to make in reconciliation and healing. SHN is hiring an Indigenous Patient Navigator (IPN) who will provide culturally relevant service navigation, counselling, support and advocacy for First Nation, Inuit and Metis and Urban Indigenous (FNIMul) patients and families. A key feature of this role is providing culturally relevant counselling, support, and advocacy for FNIMuI patients and families by facilitating and coordinating access to services, addressing cultural/spiritual needs, and networking with FNIMuI and non-FNIMuI partners

Reporting to the Director of Chronic Disease Management, the IPN supports integration with existing navigational and advocacy support currently provided by local community organizations and regional partners. The IPN will also have a broader scope in terms of community engagement, supporting an Indigenous Advisory Committee, policy development, health literacy, and other building blocks to develop an Indigenous Health Program at SHN.

Responsibilities:
- Be accountable for providing support services to FNIMul patients and families including:
- Facilitating and supporting access to traditional Indigenous health practices
- Connecting FNIMuI patients and families to spiritual and emotional supports (which may include community supports, group supports, traditional healers, Elders and Knowledge Keepers)
- Liaise with clinicians providing direct care to FNIMul patients and families to support culturally safe care
- Connecting FNIMul patients and families to local community resources
- Inform FNIMul patients and families of internal mechanisms for reporting opportunities for improvement in their care and service.
- Provide support and advocacy for FNIMul patients and families who wish to raise a concern in their care by connecting with resources the hospital has available (i.e. Patient Care Manager, Social Worker, Patient Relations)
- Assist in the analysis of these concerns to aid in quality improvement
- Build relationships with FNIMul organizations and Indigenous leaderships within the Scarborough community
- Guide and inform engagement between SHN and FNIMuI organizations and communities to support the advancement of an Indigenous Health Program at SHN that reflects FNIMuI community's needs and priorities, including the implementation of initiatives to support cultural safety for FNIMul patients, families and communities.
- Advise on creation and collaborate to ensure appropriate implementation of cultural safety training
- Advise on enhancing space at SHN to be more welcoming for FNIMul patients and their families
- Advise on health literacy initiatives related to Indigenous health
- Identify opportunities to engage with Indigenous Knowledge Keepers and facilitate this engagement
- The IPN will lead the development of a collaborative of SHN health care providers/partners to develop policies, procedures and guidelines that support Indigenous healing, customs and practices
- Facilitate and co-lead an Indigenous Advisory Committee at SHN.
- Collaborative development of goals and measures to track progress on initiatives and the evolving Indigenous Health program at SHN

Requirements:
- Lived experiences as an Indigenous person
- Strong knowledge of the history of Indigenous Peoples (First Nations, Inuit, Metis and Urban Indigenous) and the impact of intergenerational trauma.
- Experience working with FNIMul communities and organizations.
- Familiarity with health system navigation models and principles.
- Extensive knowledge of Indigenous people's language, culture, customs and traditions.
- Excellent communication skills (both written and oral).
- Superior interpersonal, collaboration, facilitation, and negotiation skills.
- Strong problem-solving abilities.
- Demonstrated ability to work effectively in a multi-disciplinary team environment.
- Demonstrated leadership, initiative, and the ability to facilitate change.
- Proficient in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook, MS TEAMS) and Zoom
- Successful completion of a Bachelor's degree in Health, Education, Social Science or equivalent experience is required.

Registered health professional designation - Preferred
